Knighton \u201cinsan plateletlerinden elde edilen ve topikal olarak uyguland\u0131\u011f\u0131nda lokal olarak etki g\u00f6steren b\u00fcy\u00fcme fakt\u00f6rlerinin\u201d kullan\u0131m\u0131n\u0131 tan\u0131mlam\u0131\u015f ve \u00f6zellikle kronik yaralarda &nbsp;doku yenilenmesini tetiklemek amac\u0131yla ven\u00f6z kandan elde edilen, b\u00fcy\u00fcme fakt\u00f6r\u00fc salg\u0131layan plazma h\u00fccrelerini kullanman\u0131n ne kadar ilgin\u00e7 olabilece\u011fine dikkat \u00e7ekmi\u015ftir. O tarihte Knighton polimer bazl\u0131 bir separat\u00f6rjel i\u00e7ermeyen laboratuar t\u00fcplerini PRP \u00fcretmek amac\u0131yla kullanm\u0131\u015ft\u0131r. (Knighton et al., 1982; Knighton et al., 1986)<\/p>\n\n\n\n<p>Maksilofasiyal cerrahide Robert E. Marx mandibular s\u00fcreklilik defektleri i\u00e7in kemik greft rekonstr\u00fcksiyonunda PRP\u2019nin kemik mat\u00fcrasyon oranlar\u0131 ve kemik yo\u011funlu\u011fu \u00fczerindeki etkisini de\u011ferlendirmi\u015ftir, greftlere eklenen PRP\u2019nin kemik formasyonunda art\u0131\u015fla sonu\u00e7land\u0131\u011f\u0131n\u0131 g\u00f6stermi\u015ftir (Marx et al., 1998)<\/p>\n\n\n\n<p>Bu \u00f6nc\u00fc uygulamalardan sonra PRP kullan\u0131m\u0131 t\u00fcm d\u00fcnyada spor hekimli\u011fi, ortopedi, kalp damar cerrahisi, estetik t\u0131p, maksilofasiyal cerrahi, plastik cerrahi, oftalmoloji ve jinekoloji alanlar\u0131n\u0131 i\u00e7erecek bi\u00e7imde yay\u0131lm\u0131\u015ft\u0131r. &nbsp;<\/p>\n\n\n\n<p><strong>PRP Uygulamas\u0131n\u0131n Teknolojik Geli\u015fimi<\/strong><\/p>\n\n\n\n<p>Halen CE onayl\u0131 ve\/veya FDA (Food and Drug Organization) taraf\u0131ndan onaylanm\u0131\u015f, 8 ila 60 ml aras\u0131ndaki ven\u00f6z kandan santrif\u00fcjleme y\u00f6ntemiyle PRP haz\u0131rlayan birka\u00e7 medikal gere\u00e7 vard\u0131r. Bununla birlikte farkl\u0131 ticari markalar olarak \u00fcretilen bu gere\u00e7lerin kar\u015f\u0131la\u015ft\u0131rmal\u0131 performanslar\u0131 \u00f6nemli farkl\u0131l\u0131klar g\u00f6stermektedir.<\/p>\n\n\n\n<p>Neredeyse t\u00fcm gere\u00e7lerde al\u0131nan kan premat\u00fcre bir koag\u00fclasyonu \u00f6nlemek i\u00e7in bir antikoag\u00fclan madde ile kar\u0131\u015ft\u0131r\u0131lmakta ve hemen santrif\u00fcj i\u015flemine sokulmaktad\u0131r. Bu gere\u00e7lerin tamam\u0131 kan\u0131 bile\u015fenlerine ay\u0131rarak y\u00fcksek d\u00fczeyde platelet i\u00e7eren bir plazmay\u0131 izole etmek olsa da her bir gere\u00e7 farkl\u0131 i\u015flevlere sahiptir. Spesifik olarak bu gere\u00e7ler (ya da medikal cihazlar) ve uygulama protokolleri plazmay\u0131 ne \u015fekilde izole ettiklerine (kan bile\u015fenleri aras\u0131na mekanik bir bariyer koyarak veya koymadan), kan toplama gerecinin tipine ve i\u015flevine, santrif\u00fcj i\u015fleminin say\u0131s\u0131na ve s\u00fcresine, istenen ba\u011f\u0131l santrif\u00fcj g\u00fcc\u00fcne ve di\u011fer \u00fcretim s\u00fcre\u00e7lerine g\u00f6re farkl\u0131l\u0131klar g\u00f6sterir. Bu farkl\u0131l\u0131klar haz\u0131rlanan PRP preparatlar\u0131n\u0131n plazma hacmine, platelet konsantrasyonlar\u0131na, b\u00fcy\u00fcme fakt\u00f6r\u00fc miktarlar\u0131na ve elimine edilen beyaz ve k\u0131rm\u0131z\u0131 h\u00fccre say\u0131s\u0131na g\u00f6re farkl\u0131l\u0131k g\u00f6stermelerinin nedenidir (Mazzocca et al., 2012). Dr Jane Fitzpatrick\u2019in yay\u0131n\u0131nda tan\u0131mlad\u0131\u011f\u0131 \u015fekilde (Fitzpatrick et al., 2017) PRP preparatlar\u0131 klasik olarak iki gruba ayr\u0131l\u0131r: \u201cbuffy coat\u201d veya l\u00f6kosit y\u00f6n\u00fcnden zengin PRP veya plazma veya l\u00f6kosit y\u00f6n\u00fcnden fakir PRP.<\/p>\n\n\n\n<p>\u201cBuffy coat\u201d sistemleri plateletleri beyaz kan h\u00fccreleri d\u00fczeyinde yo\u011funla\u015ft\u0131racak (Buffy coat katman\u0131) santrif\u00fcj protokolleri kullan\u0131rlar. Bu kitler tipik olarak ven\u00f6z kan\u0131n 3 ila 8 kat\u0131, hatta daha fazlas\u0131 platelet yo\u011funlu\u011funa sahip PRP preparat\u0131 \u00fcretirler. Bununla birlikte \u00fcretilmi\u015f olan PRP y\u00fcksek miktarda beyaz kan h\u00fccresi (ven\u00f6z kan\u0131n 2 ila 7 kat\u0131 aras\u0131nda) ve de\u011fi\u015fen miktarlarda k\u0131rm\u0131z\u0131 kan h\u00fccresi i\u00e7erirler. Bunun sonucunda olu\u015fan PRP alt\u0131n sar\u0131s\u0131 renktedir, platelet konsantrasyonlar\u0131 ise ven\u00f6z kan\u0131n 1,5 ila 3 kat\u0131 yo\u011funlu\u011funa ula\u015f\u0131r (Filardo et al., 2012).&nbsp; Plazma \u00fcreten bir \u00e7ok medikal gere\u00e7 yava\u015f bir santrif\u00fcj prosed\u00fcr\u00fc uygular; PRP ve di\u011fer kan bile\u015fenleri aras\u0131nda herhangi bir fiziksel bariyer yoktur. Bu nedenle plazma kaynakl\u0131 PRP\u2019nin h\u00fccresel bile\u015fimi operat\u00f6r\u00fcn el becerisine ba\u011fl\u0131d\u0131r ve ayn\u0131 \u015fekilde tekrarlanamaz.
